New Student Registration
Students under the age of 18 must be accompanied
by their parent or legal guardian during the enrollment process.

To report an absence, please call the Absence Reporting Line at (602) 764-5012.

 

The following information must be provided at the time of registration:

  1.   Proof of Residence   Two of the following must be submitted:

· Current Utility Bill (Gas, Electric, Water)

· Title Papers

· Purchase Contract

· Lease/Rental Agreement (must be on letterhead of rental company)

· Notarized letter of homeowner if your family is living with another family. 

   

2.   Proof of Custody or Guardianship One of the following must be submitted:

· Custody Papers

· Arizona Court Appointed Guardianship Papers

· Documentation from the Superior Court of Arizona showing the pending court date for your Guardianship hearing. Final papers must be provided within one week of the hearing date.

 

3.   Birth Certificate All students must have a Birth Certificate on file. 

 

4.   Withdrawal Papers/Forms -  Students need an official withdrawal document and transfer grades if enrolling during the school year.

 

5.   Unofficial Transcript - From all previous high schools attended.

   

6.   Test Scores -  AIMS, TERANOVA, or out of state test scores.

 

7.   Eigth Grade Diploma -  For incoming Freshman/9th grader.

   

8.   Attendance and Discipline Records - From previous high school(s).

 

9.   Immunization Records - Students must have proof of immunizations before enrolling. (Immunization card, document signed by physician, or immunization record from former school with signature). MMR after 1980, TD/DT within the last 10 years.

 

10.  Approved TE and Variance form - If previous school was a charter school.  

 

 

11.  Approved Magnet TE - If entering a Magnet Program.

 

  If you need to receive records from another school, please arrange to have them faxed to the Registration Office.

 

How to change your address or phone number:

When notifying us of a change of address, please provide the Registration Office with a copy of a current utility bill (SRP, SW gas, City of Phoenix, Qwest) showing that your residence is within South Mountain High School's attendance zone.

If you have moved outside of South Mountain High School's attendance area and remain in PUHSD attendance zone, an Inter-District Transfer-Special Request (T-SR) form must be submitted.

If you have moved outside of the Phoenix Union High School District attendance boundaries a Variance form must be submitted.

 We have a website that makes it easy for parents to check their student's grades and attendance on a daily basis.  Our teachers upload their student data to Parent Link so that parents can always stay on top of their students.
